If you know that your data was breached by Well, please complete the form here to notify the PDA that you wish to be included in any collective action.
Alternatively, if you:
- Work for Well pharmacy at present or are a locum for them
- Have previously provided locum services or worked as an employee for Well, the Co-operative Pharmacy or one of its predecessors, in the past
- Or are not sure whether you have previously worked for Well or one of its predecessors
and you want to find out whether your data was breached in this incident, please complete the Subject Access Request form available here, and email it to Well at: raisingconcerns@well.co.uk.
You can complete the survey here to let us know you are interested in being part of the claim whilst waiting for a response from Well, and once you have been informed by Well that your data has been breached, please complete the survey again to update us.
